Am I eligible?

You may be eligible if you are:

  • A resident of the United States or Canada
  • A pregnant woman diagnosed with ulcerative colitis who has either:
    • Taken ZEPOSIA® (ozanimod) during pregnancy or within 3 months prior to pregnancy OR
    • Taken other ulcerative colitis medications during pregnancy or just prior to pregnancy*

*Please note: If you have taken sphingosine-1-phosphate (S1P) medications (e.g., Gilenya®, Mayzent®, Ponvory) or methotrexate during pregnancy or just prior to pregnancy (within a month of last menstrual period), you may not be eligible to participate. The registry staff can help determine your eligibility if you have taken these medications.

The information you provide on this form will be used to contact you for the purposes of assessing your eligibility to participate in the ZEPOSIA UC Pregnancy Registry. This information will be held by PPD and not shared with the study sponsor (Bristol Myers Squibb) or any other third party without your permission. In the event you choose not to enroll, Registry staff will request permission to share your contact information with Propharma (a safety vendor hired by Bristol Myers Squibb), in order for them to contact you for safety follow up about your pregnancy. If you do not provide permission, and do not enroll in the Registry your contact details will be deleted.
